Continuum of Care
As the lead decision making body for the Fort
Worth/Arlington/Tarrant County Continuum of Care, TCHC is
responsible for strategic planning and coordination functions such
as:
- Support grantees to meet federal requirements for U.S.
Department of Housing and Urban Development (HUD) Continuum of
Care (CoC) Homeless Assistance Programs: Supportive Housing
Program, Shelter Plus Care, Emergency Shelter, Homeless Prevention
and Rapid Rehousing grants
- Manage the Homeless Management Information System (HMIS) and
HMIS reporting to support all CoC grant recipients
- Plan, review, monitor, coordinate, and advocate for homeless
services and the CoC
- Assist in the implementation of 10-year homelessness plans for
the City of Fort Worth and Arlington
- Provide education to the community on issues, statistics, and
composition of homelessness in our community
- Provide an avenue for participation in planning and services by
homeless individuals
- Support the development of permanent supportive housing
beds
- Identify housing barriers and promote access to affordable
housing for the homeless
- Identify service gaps and plan to meet the evolving needs of
the homeless
