Service Provider Coordination
One of the main purposes of the lead agency in the CoC is to
"manage a year-round planning effort that addresses the identified
needs of homeless individuals and households, the availability and
accessibility of existing housing and services, and the
opportunities for linkages with mainstream housing and services
resources" (http://www.hudhre.info/documents/CoC101.pdf).
Because TCHC works with over 100 agencies, there is a need for
leadership in interagency collaboration. We meet that need through
monthly Service Provider Roundtable and General Meetings,
committees, training, events, and special projects, such as the
Navigation of the Healthcare System and the Centralization of HMIS
Scan Card Issuance (see the Event Calendar).
TCHC is involved in every aspect of homelessness in our
community in order to ensure we meet HUD goals, meet our own goals
within the CoC Grant, and meet the needs of our clients.
