HMIS Scan Cards

In order for anyone to stay at the day or night shelters in Tarrant County, one must present the intake specialists with an HMIS Scan Card, also called a tuberculosis (TB) or identification card.

These cards are created at shelters during these times:

1331 E. Lancaster Cards are issued to UGM residents at intake and for guests after the meal. UGM will make up to 10 cards at each meal.  Staff makes a list as guest enter the building of individuals needing a card. If needed guest are given a data blitz form to complete.  After the meal guest are instructed to come to the front desk to have their card made or updated.

Clients are directed to the Tarrant County Health Department only for follow up appointments specifically assigned by the County TB staff.

SCAN CARD Instructions for Shelter Staff

New clients should be first entered into the ClientTrack HMIS system to collect basic client information.

A new card is initially issued as a YELLOW TEMPORARY card with an expiration date to allow the client 30 days to take the next scheduled TB test and TB skin read. When the results are verified by the health department, client records are updated and shelter staff can issue a permanent WHITE card to expire one year from issue.

TB testing is a two part process: once the test is applied, it MUST be read 48 to 72 hours later.
